For Employees of Businesses Located in an Ohio City
Use this form if the following criteria are met:
  • Your employer withholds taxes for an Ohio city
  • Your company's city tax rate is higher than your residence city rate.
  • You spent a number of days working outside the city where your company is located.
    • This tool can be submitted (to the city where your company is located) to receive a refund for taxes over-paid to that city. Attach this tool to the city's 'Application for refund of amounts overwithheld'. Your employer will probably need to sign the application.
